 ينبغي القول أنّ معظم الرجال لا يعانون من أعراض وعلامات محددة تدل على الإصابة بالعقم سوى عدم القدرة على الإنجاب..

 إلا أنّ بعض الرجال قد تظهر لديهم بعض الأعراض الدالة على العقم، مثل : 

- ضعف الانتصاب،- أو خروج كميات صغيرة من السائل أثناء القذف، - أو انخفاض الرغبة الجنسية، - أو الشعور بالألم في الخصيتين أو انتفاخهما، - أو الإصابة بعدوى الجهاز التنفسي المتكررة، -أو تضخم الثدي لدى الرجل أو قلة نمو الشعر على الوجه أو الأجزاء الأخرى من الجسم...

 ومن الجدير بالذكر أنّه عند تأخر الإنجاب، يتم تشخيص العقم لدى الرجال من خلال إجراء مجموعة من الفحوصات بهدف التعرف على المسبب للعقم ومن هذه الفحوصات : 

- الفحص البدني ومراجعة التاريخ الطبي للمريض.

- تحليل السائل المنوي.

- تصوير كيس الصفن بالأشعة فوق الصوتية.

- فحص الهرمونات.

- تحليل البول بعد القذف.

- الإختبارات الجينية.

- خزعة من  الخصيتين.

- التصوير بالأشعة فوق الصوتية  عبر المستقيم. 

- اختبارات وظائف الحيوانات المنوية المتخصصة.
